Report and Resolve a Behavior or Disciplinary Incident

Report an incident with a title, description, and severity, optionally linked to a student, then mark it resolved with notes.

For School administrators, Teachers, Staff. Updated 2026-09-13.

Overview

Incident records can include severity, description, status, attachments, comments, and assigned users. Teachers, employees, accountants, and admins can be assigned to follow up, and assigned users can access the incident. The list can be filtered to incidents assigned to the current user.

Resolved incidents are read-only for edits, so follow-up context should be added as comments instead.

Steps

  1. Open Incidents and create or open an incident record.
  2. Enter the incident details, severity, student or staff context, and any required description.
  3. Upload supporting attachments when evidence or documents are available.
  4. Assign the incident to the staff members who should follow up.
  5. Use Assigned to me to focus on incidents where the signed-in user is an assignee.
  6. Resolve the incident when complete, then add comments for any later notes instead of editing the resolved record.

Common problems

A resolved incident cannot be edited

Resolved incidents are read-only; add a comment for follow-up details.

An assigned user cannot see the incident

Confirm the user was saved as an assignee and belongs to the school.

Attachment upload is rejected

Use only relevant evidence files and stay within the incident attachment limit.
